Change of data in the travel document / identity card

Change of names or surname

EU citizens or their family members are required to report a change in their names or surname within 15 working days of this change occurring.

The foreign national must report this to the MOI office based on his/her address on the relevant application form.

If a foreign national changes their names or surname and submits a travel document/identity card with the new names or surname, then, upon request, the MOI issues a new residence card, the old residence card is taken from the foreign national.

When announcing these changes, the foreign national provides:

The change can be reported on behalf of a foreign national by a different person on the basis of a power of attorney. The power of attorney must demonstrate that it was signed for this specific purpose. The signatures on the power of attorney do not have to be notarized. If the change is not reported by the foreign national in person and it is necessary to issue a new residence card, the foreign national will be invited to collect it in person. In the case of a biometric card, he/she will be asked to attend to take the data necessary for the production of the new card and subsequently to receive the new card.

If a foreign national is prevented from reporting the change within the legally required period due to reasons outside of his/her control, he/she must report the change within 3 working days of these reasons ending. The foreign national must prove these reasons. If the change is not reported within the legally required period and there is no obstacle that is outside of the foreign national’s control, the delay in reporting a change will be judged as a violation and a fine can be imposed.
 

Change to your personal status

EU citizens or their family members are required to report a change in their personal status within 15 working days of this change occurring.

The foreign national must report this to the MOI office based on his/her address on the relevant application form.

When announcing these changes, the foreign national provides:

  • a document on the change in the personal status (e.g. a marriage certificate or a court’s final judgement on a divorce),
  • travel document / identity card,
  • his/her residence card.
     

If the document on the change in personal status is issued by a state other than the CR, it is necessary to ensure an official translation into the Czech language.

The change can be reported on behalf of a foreign national by a different person on the basis of a power of attorney. The power of attorney must demonstrate that it was signed for this specific purpose. The signatures on the power of attorney do not have to be notarized.

All the administrative proceedings are done on the spot.

If a foreign national is prevented from reporting the change within the legally required period due to reasons outside of his/her control, he/she must report the change within 3 working days of these reasons ending. The foreign national must prove these reasons. If the change is not reported within the legally required period and there is no obstacle that is outside of the foreign national’s control, the delay in reporting a change will be judged as a violation and a fine can be imposed.
 

Other changes

EU citizens or their family members are required to report a change connected to the data in their residence card within 15 working days of this change occurring.

The foreign national must report this to the MOI office based on his/her address on the relevant application form.

When announcing these changes, the foreign national provides:

  • a document confirming the change,
  • a travel document / identity card,
  • his/her residence card,
  • if there is a change in the data on the laminated side of the residence card, then 2 photographs.
     

If a new residence permit/ residence card/ permanent residence permit/ permanent residence card/ registration certificate is issued on the basis of the reported change, an administrative fee must be paid when reporting the change.These changes can be reported on behalf of a foreign national by a different person on the basis of a power of attorney. When changing their appearance, EU citizens or their family members are obliged to apply for a new residence permit without undue delay.
