Transfer of Visa Sticker/Collection of Residence Permit

A holder of a long-term visa/long-term residence permit/permanent residence permit whose passport has expired/has been lost/stolen/residence card has been lost/stolen while abroad can file an application at the Czech Embassy in Islamabad, if the visa was originally issued there, for the so-called transfer of a visa sticker.
A holder of a long-term residence permit or visa can apply for a visa for the purpose of collecting a residence permit (D/VR), if the validity of a long-term residence permit or visa expires in the course of proceedings on a legitimate newly filed application or after approving applications for a long-term residence permit filed at an embassy. This visa enables the applicant to return to/enter the Czech Republic.
REQUIREMENTS
2. new passport
3. photo
4. copy of the old passport and visa – if available, proof of possessing a residence permit, other as relevant to the situation
